By signing this consent form, you, and any of your children listed below, agree to participate in photos and / or video being taken for Radical Fields Inc (RF).
These images will be used by RF, for an indefinite period of time, and may be reproduced in publications, brochures, social media channels or on RF’s website. The images will be used to promote a range of initiatives and programs and may be disclosed to third party organisations for non-commercial publicity purposes.

CONSENT:
(a) I, the undersigned, agree to grant a non-exclusive licence to RFand any person authorised by RF at its discretion, to copy or reproduce such material without acknowledgement of myself and without the entitlement to any remuneration of compensation now or in the future. I agree that I will have no further rights to the images including moral rights and copyright.
(b) RF agrees not to use any image in a manner that may be deemed adverse or defamatory to the person signing this form. RF further agrees that it will not use the imagery for any political or commercial gain.
(c) RF agrees to only identify a child by reference to the child’s first name.
